Medical Center Account Request

To request access to Medical Center applications, please submit the Account Request Form (ARF). In order to proceed you will need the UCSF ID of the person who needs access. 

Once the ARF has been approved, you will receive an email from Service-now with the ticket number of your request.

If this account will require elevated access, such as access to a Medical Center server, it is required that the ARF be requested for an admin account. Additionally an elevated account access request form needs to be submitted for review. That form is located here:

Please note that the Elevated Account Access Request form must be signed by the individual for who's account is requesting the access.
