To request a change in APeX, please fill out the form in the link below.

You will need the following information: your login department, the name of your superuser or department manager, and the Apex application in which you want the change.

Don’t know your login department? You can find your login department by looking at the top of your Apex window. After the word “Hyperspace”, you should see your login department. Ex: Hyperspace – 10LS CVT – UCSF Production – John D. – “10LS CVT” is the login department.

Someone will contact you within 24 hours confirming your request has been received and may seek additional information.
