Overview
The following steps describe how to complete and submit an onboarding request form for a new non-employee, either clinical or non-clinical. Complete the form, making sure to populate all required fields designated by an asterisk (*).

Step1
Helpful tip
Before creating a new identity for incoming affiliates, check to see if they already have one as a previous affiliate, student, or employee. Navigate to the De-Duplication web app and search by name, date of birth, or personal email address to locate an existing account and help prevent duplicate UCSF identities.

Select the applicable form on MyAffiliate Dashboard
From the MyAffiliate dashboard, select either Non-Employee Onboarding Form – Non-Clinical or Non-Employee Onboarding Form – Clinical. Insert Image 4 For clinical non-employee onboarding, select the non-employee type and service area before clicking Submit.

When the selected form loads, populate the fields outlined below:

- Personal Information: Enter non-employee's first name, last name, and date of birth.
- Contact Information: Enter personal email and personal cell number (e.g., 415-555-5555).
- Work Address: Select physical work location type (either domestic or international).
- Home Address: Enter the provider’s personal address information. While this information is not required, entering the home address assists with accurate record keeping.
- Job Details: Enter the following required job details:
- Organization: The name of the vendor/supplier the non-employee belongs to. If the name does not come up in the pre-defined search list, select Other.
- Sponsor: The sponsor is the person accountable for the non-employee's UCSF identity and is asked to approve this and future lifecycle requests.
- Affiliation: The official association or relationship with UCSF for the non-employee. For example: CONFER (Conference Attendee), AFFCTH (Affil Cardio Rad Technologist), ACADAP (new academic).
- Start Date: The date the non-employee’s account should be available and active. The start date cannot be in the past.
- End Date: The date the affiliation and/or job details are projected to end for the non-employee.
- Cost Center Information: The 6-digit code and department name associated with the non-employee for administrative supervision (e.g., 200068 Radiology).
- Department Information: The 6-digit code and department name associated with the non-employee. For example, 200068 Radiology. NOTE: Cost center and department will frequently be the same value, but not always.
Step4
Enter the specific services the non-employee will provide.
Required fields in this section are: Active Directory Account Domain and Email Required.
- Academic Personnel: ACADAP affiliations must be provisioned only to the CAMPUS Active Directory. If an ACADAP user needs an account on the Medical Center domain, they need to request it via the ARF form instead of MyAffiliate. This applies only to ACADAP affiliations.
- Human Resources: NEWEMP affiliation accounts must be provisioned to the domain that aligns with the business unit. Users working for a Campus business unit must have only Campus AD accounts. Employees working for a MedCenter business unit must have only UCSFMC AD accounts. If, during onboarding, both is selected, then the MedCenter account will be deactivated once the user reaches their NewEmp date.

With the desired information added, click Submit.
This request will be sent to the Identity Sponsor for review.
